For all studies which will be using the Hugh Sinclair Unit of Human Nutrition, please also complete the correct Consent Form, Adverse Events Form and Medical and Lifestyle Questionnaire.

If your project involves the taking of blood samples, you should also refer to the information sheet and consent form for the taking of blood samples - Annex D1 (PDF-83kb)

Process

Initially YOU MUST speak with your Department Ethics Rep, ensuring that they are satisfied for you to submit an application - Rainer Cramer (Chemistry), Julie Lovegrove (Food) and Amelia Hollywood (Pharmacy) and for them to decide whether it should go down the Internal Approval route or be submitted to UREC. IT IS NOT FOR YOU TO MAKE THAT DECISION YOURSELF

Send in a word document, via email to Barbara Parr b.l.parr@reading.ac.uk a minimum of at least 2 weeks prior to any UREC submission deadlines completed copies of:-

  • Ethics Application Form
  • appropriate Consent form
  • Any information sheet for participants
  • any supporting papers

These must be sent a minimum of at least 2 weeks prior to any submission deadlines. She will arrange for it to be reviewed by an internal reviewer. Once reviewed satisfactorily, the PI to insert the name of the internal reviewer and sign electronically, then arrange for the Head of Department to sign electronically and return to Barbara Parr b.l.parr@reading.ac.uk.

If the application is to go to UREC then please also arrange for the Ethics Chair to sign (Julie Lovegrove or Bob Rastall) and if using the Hugh Sinclair Unit, then Kelly Jarrett, the Hugh Sinclair Manager should sign.

Please then forward the final version to Barbara Parr b.l.parr@reading.ac.uk by email so she can arrange for it to either be submitted to UREC or given internal approval.
